I often lie on these forms.

My email address is tied to my own domain and I don't recall filling out a "What is your name, age, sex, location" form when creating my email address. Though I did fill out something similar when creating my domain.

There was talk of the white pages in australia including the email address as well as your phone number.

But I think with the spam laws/problem that was stopped.

First, get an envelope. Write the following on it:

[FirstName] [LastName]
[Address]
[City], [State]
[Postal Code]
[Country]

Then, print the following on a sheet of paper:
