Looking for a website monitoring service that can offer the following features:

5 minute intervals
SMS Alerts (Cellphone, pager)
Multiple Alert Contacts

Anyone using one that they would recommend? TIA.

I second Nagios.  Pretty easy to set up services to watch, hosts, etc.

You can do a lot of cool stuff with it above and beyond the stuff you mentioned, like monitor databases via scripts.

You might want to try creating the service yourself (probably not worth it until you really care). Basically you just need to be able to send email.

We use here - they do everything you're asking for as well as other useful bits and pieces like page component download time analysis to help you optimise page loading. I've no idea what their service costs, but I can vouch for the quality of the service. We are a well-known (at least in the UK) brand with a fairly high-profile site, and web services syndicated to even higher profile partners with 24x7 on call support, and SiteConfidence have really helped us to maintain our service availability, as well as optimise our page downloads and monitor competitor sites.

We use Global Web Monitor's Global Up Time monitoring Service. More monitoring frequencies than anyone easy to lower monthly monitoring charges. They also have a new port security check which has worked well for us.  or  for their other services
