In 1986 Richard Feynmann wrote a report on the Challenger disaster. The report details how quality and safety was dropped gradually with each shuttle launch: just because it worked once it was considered safe.

The exact same thing happened nowdays.

Not to nitpick, but didn't he actually just write the appendix to the report (in addition to serving on the committee that wrote the report)?
