Error after Upgrade to 3.1.9

I'm getting an error only when trying to create a new bug.  I can edit bugs just fine.  The error is:

Your bug was NOT submitted. Please hit back and resubmit your bug.: 006~ASP 0177~Server.CreateObject Failed~8007007e

If I back up and resubmit the same data, the error changes to:
Microsoft OLE DB Provider for SQL Server error '80040e14'

Incorrect syntax near the keyword 'DEFAULT'.

I would make sure FogBUGZ user has full permissions on all your files in the FogBUGZ tree, then I would run the register.bat script in Accessories.  Finally I would make sure that the Anonymous User in IIS was still FogBUGZ for the FogBUGZ virtual directory and I would restart IIS.
